Addiction is complex and affects people differently. Addiction treatment at Hope Trust begins with a clinical care team developing a plan to address your specific recovery needs. Hope Trust professionals in the fields of addiction counselling, psychology, psychiatry, spiritual care, and family care collaborate to provide you with the best foundation for lifelong recovery.
Support for Continued Progress in Recovery
Prior to discharge, each client goes through a personalized and comprehensive Relapse Prevention Plan. We recognise that post treatment, early recovery is a vulnerable time. A strong continuing care (follow-up) program is important to help you stay sober longer and minimize the risk of relapse, in addition to various continuing care groups and outpatient services. As a part of the follow-up program, the client and family is expected to visit the treatment centre (or online via Skype) post discharge and review any ongoing challenges. Local support group meeting addresses are provided. Regular contact with the client is established via social media, emails and Skype.
